The Urology Services Inquiry (USI) commenced its work on 6th September 2021 and wishes to hear from the public to assist with its investigations. The Inquiry wishes to give a final opportunity to anyone who still wants to contact them to come forward.  If anyone wishes the Inquiry to know about their experiences, they are asked that they contact the Inquiry no later than 31October 2023.  They can do so by completing the questionnaire which can be found below, or by calling the Inquiry team and obtaining a hard copy of the questionnaire.  On receipt of the completed questionnaires the Inquiry will determine whether we need to hear from any further patients or family members.
