PCC Consultation Response

The PCC has published its consultation response to the Framework for Learning and Improvement from Patient Safety Incidents Consultation.

Based on our work in Advocacy and Engagement we highlighted:

  • The current deficit of trust in the existing Serious Adverse Incidents process
  • What those who have experienced an adverse incident want from a new process and what should they expect
  • There should be a robust investigation to a consistent standard
  • The need for independence and assurance throughout the process
  • The importance of Independent Advocacy Services

 

The PCC were asked by the Department of Health to facilitate an online consultation event. We wrote a report summarising the views and experiences expressed by those who participated and follow-up written responses shared with PCC. This report was shared with the Department.

The discussion was framed around the following questions:

  • What is important to you in a Patient Safety Incident Framework?
  • Based on the proposals:
    • Do you need any clarification?
    • What aspects do you welcome?
    • Do you see any gaps or areas for development?

Participants welcomed there was an acknowledgement that the current system is not working and that it requires change. They felt reporting and monitoring, support provided and consistency across Trusts on how Serious Adverse Incidents were handled were some of the areas they felt could be improved.
