PCC Statement: Serious Adverse Incident Process - in the media 7 October 2024

The Patient and Client Council (PCC) acknowledge the issues raised by the Northern Ireland Public Services Ombudman (NIPSO) regarding the Serious Adverse Incident (SAI) process, in response to a Southern Health and Social Care Trust Complaint and others that have been received in recent years.

Through our work in advocacy and engagement with those affected by serious adverse incidents, we recognise many of the issues raised by NIPSO’s report.  Many of these issues are reflected in our recently published report, which provides the PCC’s outline assessment of the current state of the Serious Adverse Incident (SAI) Review system in Northern Ireland.

The information contained in this report is based on our engagement with those affected by SAIs, and our broader organisational experience, including providing independent advocacy support in SAI reviews. You can read it via our website here.

Commenting on the SAI process and PCC’s support offer, Katherine McElroy, PCC Principal Practitioner for Advocacy said:

“The SAI process can be extremely complex and occurs at a time when families and individuals may be suffering from grief and trauma.

“It is important that the public know that the PCC offers independent advocacy support, from trained practitioners, through complaints and SAI processes. The service is independent, confidential and free to use.

“The PCC is also seeking to ensure that members of the public’s lived experiences of the SAI process are considered during the ongoing Department of Health (DOH) review.  It is critical that any review addresses significant issues that impact on public trust and confidence, including candour, openness and demonstrable learning from patient safety events.
