The Regulation and Quality Improvement Authority (RQIA) is undertaking a Review of the Learning Disability Hospital Passport in Northern Ireland. They would like to gather the views from those who have a primary diagnosis of a learning disability and are 18 years of age and over.
RQIA have created a smart survey and would like you to complete this to help them understand how the hospital passport is working for you.
This will help to:
- Find out how many people with a learning disability have a hospital passport
- Find out if the hospital passport is working for you
- Find out how the hospital passport could be made better
The questionnaire will be in smart survey format and can easily be accessed using the QR Code below or this link https://www.smartsurvey.co.uk/s/LDPassport/

Please complete the survey yourself or with the help of someone you trust.
When you click on this link you will also see a Participant Information Sheet describing the purpose of the Review in more detail. There will also be a Consent Form for you to complete.
Please submit your completed survey by 5pm on Thursday 12th June, 2025.
